Last week, VA became aware that Federal Express (FEDEX) deployed a software change in July and August 2024, that impacted the delivery of refrigerated medications that are delivered in coolers to veteran’s homes. FEDEX deployed the software change without notification, input, design, or testing by VA. The intent of the software was to identify at-risk containers and route them back to VA Medical Centers. However, the newly deployed software had a bug that erroneously caused packages not truly at risk to be sent back to VA facilities.
This may have caused a delay in medication delivery to veterans. Patients impacted by this will be contacted by VA and medication needs will be addressed. VA leadership will be holding regular meetings until the issue has been resolved and will hold FEDEX responsible for costs associated with their programming error. Patients may contact their local VA medical facility for timely resolution if they believe they have been impacted.
